How to Register
You will be asked to complete a registration form for each person registering with the Practice.
In addition, all patients will be asked to complete a health questionnaire and asked to attend for a health check with the Practice Nurse only if you are not taking any medication and with a GP and Practice Nurse if on medication.
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
In line with Scottish Government advice, please note we are only accepting patients who are outwith their current practice boundary.
The Practice area is set out below. Please check with the Practice on 01382 668189 before completing the forms to check if you are eligible to register. The practice covers the following all of DD2 1-4 then DD2 5 needs to be confirmed. DD1 needs to be confirmed.
We do not cover any other areas. Please check with the Practice if you are unsure.
It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.
Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

If you prefer you can download the registration form, via NHS inform and upload to the Practice on the form below. Please also complete our New Patient Questionnaire. If you are unable to do this please contact the Practice for further advice on how to register.
Once you have completed BOTH forms please upload these to the Practice.
Patient Right to Express Preference of Practitioner
Patients are registered with the Practice and not an individual GP.
However, to ensure continuity of care, if you wish to be seen by a specific GP please inform the Reception staff of the GP and they will record this on your computer held record.
However, not all the doctors in the practice provide all services and specific doctors may not be immediately available.
The reception staff can advise you of the individual GP’s special interests.
