Next time you are in the practice please complete a form in the waiting room to ensure we have your up to date contact details. There is a box beside reception desk to put it in, no need to queue at reception. If you have dependants please note their names too so we can check their records. If your young person is almost 16 years old or has turned 16 years old recently, please discuss with them about updating their records where appropriate.
